骨移植, 上顎

Key points

骨高径5～6 mm、骨幅6 mm以下の萎縮上顎無歯顎に対しては、インプラント埋入前に骨増生術を行う必要があります。
前歯部では、垂直的および水平的骨増生のため自家骨ブロック移植片を使用します。
上顎臼歯部では、上顎洞底挙上術が垂直的骨増生のための第一選択となります。
上顎洞底挙上術を行う場合は、インプラントを即時埋入または遅延埋入します。
上顎洞底挙上術において、無機ウシ骨移植材は自家骨と同等の有効性を示すことが報告されています。

適応症

上顎無歯顎では、隣接する解剖学的構造（鼻孔底、上顎洞）のためにインプラント挿入が制限されることがあります。骨高径5～6 mm以下および骨幅6mm以下の場合は、インプラント埋入前に骨増生術を行う必要があります。

上顎前歯部

上顎前歯部における垂直的または水平的骨増生のための第一選択は、自家骨ブロック移植です（図1）。代替として同種骨ブロックを用いた移植術にも関心が集まっていますが、未だエビデンスが少ないため、現時点では推奨されません。顎堤骨移植後 4～6ヵ月の治癒期間を置き、2回目の手術でインプラントを埋入します。

上顎臼歯部

上顎臼歯部に骨増生を行う場合は、上顎洞底から洞粘膜を挙上し、底部に骨移植を行います。上顎洞底挙上術を行う場合は、インプラントの初期固定に応じて、即時または遅延インプラント埋入による1回法または2回法により行うことができます。残存歯槽骨の高さが≧3～4 mmの場合は、上顎洞の骨移植と同時にインプラントを挿入します。高さ＜4 mmの場合は、十分な初期固定が得られない可能性が高いため、2回法が推奨されます。4～6ヵ月間の治癒期間をおき、遅延インプラント挿入を行います。

骨増生後の上顎洞に挿入したインプラントの荷重時期は、4～6ヵ月後が推奨されます。

代用骨

上顎洞の骨移植に関しては、ブロック型移植材と比較して癒合特性が優れているという理由から、粉砕骨材が使用されています。上顎洞底挙上術には、さまざまな粉砕骨材および自家骨移植片を使用することができます（図2）。インプラントの残存率および新生骨に関しては、無機ウシ骨移植材（ABB）は粉砕自家骨と差がないことが報告されています。重要な所見として、無機ウシ骨は非吸収性の代用骨であり、新生骨と複合体を形成するため、経時的な骨吸収を低減し、インプラントの長期固定が得られる可能性があります。上顎洞底挙上術に関しては、さまざまな術式が提案されていますが、現時点では術式間の優劣を示すデータは確認されていません。

外方アプローチ

My mom is a healthy 64 year old woman. She is and upper edentulous patient who needs a sinus lift and needs horizontal and vertical BRG in the anterior region.
