It is imperative that the peri-implant mucosa be inspected carefully to ensure no residual impression material is retained in the sulcus. Cementation of crowns on abutments can also result in cement being expressed and retained well below the mucosa. Great care needs to be exercised not to express cement apical to the crown being cemented.
Oral implants, as well as natural teeth, have to pierce the oral mucosa and enter the oral cavity to be functionally useful. This penetration establishes a transmucosal communication between the inner parts of the body and the external environment for multiple toxic micro-organisms or substances.
In the anterior maxilla, it often is necessary to fabricate custom abutments to create an angled transition from the long axis alignment of the implant to the desired long axis of the intended crown. When a metal abutment is planned, it is important that the implant be positioned incisocervically below the peri-implant mucosa so none of the structure of the metallic abutment is visible. To achieve this, measurements of the mucosal height incisal to the implant platform should be made, so the desired submucosal level can be communicated to the dental laboratory.
When placing single dental implants, the use of a conventional surgical flap that includes the two interdental papillae has been compared to a modified design that did not sever the two interdental papillae. With the modified flap design, at least one millimeter of the papillae adjacent to the natural teeth was allowed to remain in place. A reduction in crestal bone loss was noted and this enhanced bone preservation could improve the esthetic result achieved.
